Your client intake forms must be completed prior to session beginning. In order to make this as simple as possible you will find these 3 forms listed below.  Please print them from this site, complete, and bring completed forms with you for your first appointment.  If you are unable to print from this site you can choose to arrive for scheduled appointment 15-20 minutes early and complete your forms at the office. The session will not start until completed forms are received.

Please pay close attention to Fee Acknowledgement form. The office has a strict policy in regards to tardiness, re-scheduling, and cancellations. You as the client can always choose to cancel or reschedule or discontinue sessions. However, a minimum of 24 hour notice must be given to the office or you will be charged for your session.

Any issue which may have a medical origin will require a referral from your primary physician. I will be happy to fax that request to your physician. Please bring your physician name, address, phone number, and fax number with you.
